INSTRUCTIONS WHEN APPLYING FOR MEDICALLY ASSISTED REPRODUCTION FOR WOMEN IN THE AGE GROUP 50-54 YEARS
REQUIRED DOCUMENTS INCLUDE:
1. A simple handwritten or electronic application with the date and signature of the applicant, accompanied by a copy of identification (identity card or passport).
2. The opinion of a cardiologist or pathologist certifying that: a) “the interested party is fit to undergo medically assisted reproduction, including the use of hormonal preparation and b) “she is deemed capable of conceiving”.
3. The opinion of a gynecologist certifying that a) “the interested party may undergo medically assisted reproduction which may include the use of hormonal preparations and b) “she is deemed capable of conceiving”.
4. The opinion of a physician on the breast-health of the applicant certifying that: “the interested party, based on her current state of health, can undergo medically assisted reproduction which may include the use of hormonal preparations”.
All certifications should be recent (within the last six months), legible and clearly indicate the date of the examination, the name of the applicant and be signed and stamped by the relevant physician.
All documents should be sent to the National Authority I.Y.A. to the email email@example.com EXCLUSIVELY as pdf file format. (Any other file format (jpg, jpeg, etc) will not be accepted.
